Associate Degree Nursing - Spring 2015 Class Confirmation

In order to confirm your acceptance of the tentative offer of admission for the spring 2015 class of the Associate Degree Nursing (ADN) program, please take a few moments to complete the Confirmation Form below.  Completing this form will ensure that you will receive information regarding the mandatory orientation session that will be held during the spring semester 2014.

Confirmations must be received by 9:00 a.m., July 23, 2013.

In order to maintain your eligibility for program admission, you must: 

  • Confirm your acceptance by the above deadline.  If we have not received your confirmation by the deadline, your seat will be given to the next eligible applicant.
  • Maintain a 2.5 comprehensive grade point.  If your grade point average falls below the minimum requirement, you will be ineligible for program admission. You will be removed from the fall class, and you will have to achieve a 2.5 grade point average in order to reapply to the program.
  • If you have outstanding transcripts, they must be received by the Records Office by January 5, 2014—this deadline is also applicable to any changes that may be made to one’s grades (i.e. – Incomplete grades, petition for grade change, etc.).

By submitting this form, I certify that the information that I have provided is complete and accurate to the best of my knowledge.  Additionally, I understand that this is a tentative offer of admission, and that it is contingent upon my having a 2.5 comprehensive grade point average at the time I begin the ADN program.  I further understand that I am responsible for submitting all outstanding college/university/career/technical transcripts that will impact my grade point average by the January 5, 2014 deadline; and that failure to do so may result in my removal from the ADN program.  Lastly, I understand that LCCC reserves the right to change the program curriculum, admission criteria, or admission procedures at any time without prior notification; and that LCCC is not responsible for undelivered mail (sent electronically or via the U. S. Postal Service), that I fail to receive.
