Online Health Assessment

You will need the following biometric measurements to complete the online assessment:

Height	Blood Pressure	HDL
Weight	Triglyceride	LDL
Waist Circumference	Fasting Glucose	Total Cholesterol

Full-time employees and their spouses can complete the assessment from November 12 - December 14 at http://www.medmutual.com/.
