Lorain County Community College will be holding its annual Benefit Open Enrollment from now through October 28, 2011.  Open Enrollment provides a once-a-year opportunity for you to make changes to your medical, dental, and vision insurance, flexible spending accounts, and/or AFLAC supplemental benefits, as well as add dependents to your plans.  Any changes you elect will become effective January 1, 2012.  

WHAT YOU NEED TO DO FOR OPEN ENROLLMENT:

Review your individual benefit statement, which you will soon receive in the interoffice mail, if you haven't already.

Determine what if any changes you would like to make to your benefits, i.e. change to a different medical plan, add a dependent, etc., and then complete the applicable benefit enrollment/change forms, available below.

Complete the Flexible Spending Account Enrollment form below, if you want to enroll for the first time in a flexible spending account or simply want to continue your current participation into 2012.

Complete the LCCC Full-Time Student Certification form below, if you are planning to provide dental coverage for your son/daughter, age 19 to 23 in 2012.

Complete the Spousal Insurance Certification form below, if you are planning to provide primary or secondary medical insurance coverage for your spouse in 2012. 

Complete the Health Savings Account Contribution form below, if you want to enroll in a High Deductible Health Plan with Health Savings Account (H.S.A.) for the first time or if you wish to continue contributing to your H.S.A. during 2012.

Contact the college's AFLAC representative, David (Rocky) Campana at david@campanainsurance.com or at 440-984-2531, if you want to enroll for, change, or cancel your participation in one of AFLAC's supplemental cash benefits, i.e. short-term disability, sickness indemnity plan, etc.  You do not need to contact him regarding your enrollment or continuation in a flexible spending account.  For that you simply complete the Flexible Spending Account Enrollment/Change form below. 

Submit all benefit enrollment/change forms applicable to Open Enrollment to Sharon Whitmore in the Human Resources Office (CC 209) by October 28, 2011.

If you do not need to complete any of the forms referenced above, then you do not need to do anything for purposes of Open Enrollment and your current benefit coverage will continue into 2012.

Below is a Notice that LCCC is required to provide employees each year as part of Open Enrollment:

The Women's Health and Cancer Rights Act of 1998 

In accordance with the Women's Health and Cancer Rights Act of 1998, group health plans, insurance companies and health maintenance organizations (HMOs) offering mastectomy coverage must also provide coverage for reconstructive surgery in a manner determined in consultation with the attending physician and the patient.  Coverage through Medical Mutual of Ohio's medical plans includes:

  • Reconstruction of the breast on which the mastectomy was performed.
  • Surgery and reconstruction of the other breast to present a symmetrical appearance.
  • Prostheses and treatment of physical complications at all stages of the mastectomy procedure, including lymphedemas (swelling of the hand and arm on the operated side).
