• This form is to be used by LCCC students, staff, and faculty at St. Joseph Learning Center, Wellington Center, Midpoint Campus Center, or the Elyria Growth Partnership Learning Center.
• This form is to be used for requesting library items owned by the Lorain County Community College Library.
• Please complete the form in its entirety.
• Only one item per form.
• The library item will be delivered to your residence.
