Free Application for Federal Student Aid (FAFSA) 
This online form is your first step in the financial aid process, and determines your eligibility for the Pell Grant, Federal Work Study and Stafford Loans.

Request for Scholarship Appeal
Application to reinstate a scholarship that was previously revoked.

Federal Student Aid Verification Worksheet
Form from the U.S. Department of Education that requires verification if information submitted on your financial aid application.

Satisfactory Academic Progress Appeal Form 
Application to reinstate Title IV financial aid that was previously revoked.

Consortium Agreement
Use this form if you are seeking a degree or certificate from Lorain County Community College and are enrolled at another institution. This Consortium Agreement allows LCCC to disburse financial aid based on your combined enrollment at both.

Transcript Request Information

  • Electronic Transcripts will be issued to all Ohio public college and universities.  We will not be faxing transcripts if we electronically send the transcript.  We will not be sending electronic transcripts by email to students, companies or any other college or universities. 
     
    If you requested two transcripts to be sent to a college or university and we are sending your transcripts electronically we are only going to send one transcript.
     

Forgiveness of Grades Petition
Lorain County Community College recognizes that students may not be able to overcome previously earned poor grades in order to meet new career and educational goals and/or to meet graduation requirements.  Academic Forgiveness may be granted one time only at the discretion of the Committee on Scholastic Standing.
